An Evaluation of the Efficacy of GraphoGame Rime for Promoting English Phonics Knowledge in Poor Readers

Here, we report further analysis of data drawn from a Randomized Controlled Trial (RCT) run in the United Kingdom designed to evaluate the efficacy of an adaptive software game to aid the learning of English phonics, GraphoGame Rime. We evaluate the efficacy of GraphoGame Rime for the âtop halfâ of players in the RCT, children aged 6 to 7 years who played above the group mean play progress point (95 children). We also analyze three sub-groupings of this cohort. The GraphoGame family of games in different languages was originally designed to support children at family risk of dyslexia, hence we analyzed data for the subgroup of the GraphoGame Rime children who were struggling in school and had Individual Education Plans (IEPs). Secondly, we analyzed data from the younger children in the RCT, born in the Spring and Summer months, as international studies of GraphoGame have found the strongest effects during the first year of reading tuition and our participants were in their second year of reading tuition. Finally, we analyzed GraphoGame Rime data from players in schools rated as ârequiring improvement.â Schools that are found to be ârequiring improvementâ in the United Kingdom are encouraged to use additional teaching strategies to achieve better outcomes. GraphoGame Rime is relatively cheap to acquire and easy to implement, hence if it offers significant gains over âbusiness-as-usualâ this would be a valulable additional strategy for such schools. We find that GraphoGame Rime is more effective than âbusiness-as-usualâ in developing knowledge of English phonics for all of the groupings analyzed. We conclude that the supplementary use of GraphoGame Rime in addition to ongoing classroom literacy instruction can benefit children in learning phonic decoding and spelling skills.Wellcome Trus
Survey of ultrasound practice amongst podiatrists in the UK
Background: Ultrasound in podiatry practice encompasses musculoskeletal ultrasound imaging, vascular hand-held Doppler ultrasound and therapeutic ultrasound. Sonography practice is not regulated by the Health and Care Professions Council (HCPC), with no requirement to hold a formal qualification. The College of Podiatry does not currently define ultrasound training and competencies. This study aimed to determine the current use of ultrasound, training received and mentorship received and/or provided by podiatrists using ultrasound. Methods: A quantitative study utilising a cross-sectional, on-line, single-event survey was undertaken within the UK. Results: Completed surveys were received from 284 podiatrists; 173 (70%) use ultrasound as part of their general practice, 139 (49%) for musculoskeletal problems, 131 (46%) for vascular assessment and 39 (14%) to support their surgical practice. Almost a quarter (n=62) worked for more than one organisation; 202 (71%) were employed by the NHS and/or private sector (n=118, 41%). Nearly all (93%) respondents report using a hand-held vascular Doppler in their daily practice; 216 (82%) to support decisions regarding treatment options, 102 (39%) to provide diagnostic reports for other health professionals, and 34 (13%) to guide nerve blocks. Ultrasound imaging was used by 104 (37%) respondents primarily to aid clinical decision making (n=81) and guide interventions (steroid injections n=67; nerve blocks n=39). Ninety-three percent stated they use ultrasound imaging to treat their own patients, while others scan at the request of other podiatrists (n=28) or health professionals (n=18). Few use ultrasound imaging for research (n=7) or education (n=2). Only 32 (11%) respondents (n=20 private sector) use therapeutic ultrasound to treat patients presenting with musculoskeletal complaints, namely tendon pathologies. Few respondents (18%) had completed formal post-graduate CASE (Consortium for the Accreditation of Sonographic Education) accredited ultrasound courses. Forty (14%) respondents receive ultrasound mentorship; the majority from fellow podiatrists (n=17) or medical colleagues (n=15). Over half (n=127) who do not have ultrasound mentorship indicated they would like a mentor predominantly for ultrasound imaging. Fifty-five (19%) report they currently provide ultrasound mentorship for others. Conclusions: Understanding the scope of ultrasound practice, the training undertaken and the requirements for mentorship will underpin the development of competencies and recommendations defined by the College of Podiatry to support professional development and ensure safe practice.</p
Light-Touch Interventions to Improve Software Development Security
Many software developers still have little interest in software security. To change this, we need 'interventions' to development teams to motivate and help them towards security improvement. An intervention costing less than two days' effort from a facilitator plus half a day of team effort can significantly improve that team's software security. This case study describes how this approach was used with one commercial team, and identifies its impact using Participative Action Research. With suitable improvements, the approach has the potential to help many other development teams
Multicentre cohort study to define and validate pathological assessment of response to neoadjuvant therapy in oesophagogastric adenocarcinoma.
BACKGROUND: This multicentre cohort study sought to define a robust pathological indicator of clinically meaningful response to neoadjuvant chemotherapy in oesophageal adenocarcinoma. METHODS: A questionnaire was distributed to 11 UK upper gastrointestinal cancer centres to determine the use of assessment of response to neoadjuvant chemotherapy. Records of consecutive patients undergoing oesophagogastric resection at seven centres between January 2000 and December 2013 were reviewed. Pathological response to neoadjuvant chemotherapy was assessed using the Mandard Tumour Regression Grade (TRG) and lymph node downstaging. RESULTS: TRG (8 of 11 centres) was the most widely used system to assess response to neoadjuvant chemotherapy, but there was discordance on how it was used in practice. Of 1392 patients, 1293 had TRG assessment; data were available for clinical and pathological nodal status (cN and pN) in 981 patients, and TRG, cN and pN in 885. There was a significant difference in survival between responders (TRG 1-2; median overall survival (OS) not reached) and non-responders (TRG 3-5; median OS 2¡22 (95 per cent c.i. 1¡94 to 2¡51) years; Pâ<â0¡001); the hazard ratio was 2¡46 (95 per cent c.i. 1¡22 to 4¡95; Pâ=â0¡012). Among local non-responders, the presence of lymph node downstaging was associated with significantly improved OS compared with that of patients without lymph node downstaging (median OS not reached versus 1¡92 (1¡68 to 2¡16) years; Pâ<â0¡001). CONCLUSION: A clinically meaningful local response to neoadjuvant chemotherapy was restricted to the small minority of patients (14¡8 per cent) with TRG 1-2. Among local non-responders, a subset of patients (21¡3 per cent) derived benefit from neoadjuvant chemotherapy by lymph node downstaging and their survival mirrored that of local responders
Regional differences in APD restitution can initiate wavebreak and re-entry in cardiac tissue: A computational study
Background
Regional differences in action potential duration (APD) restitution in the heart favour arrhythmias, but the mechanism is not well understood.
Methods
We simulated a 150 Ă 150 mm 2D sheet of cardiac ventricular tissue using a simplified computational model. We investigated wavebreak and re-entry initiated by an S1S2S3 stimulus protocol in tissue sheets with two regions, each with different APD restitution. The two regions had a different APD at short diastolic interval (DI), but similar APD at long DI. Simulations were performed twice; once with both regions having steep (slope > 1), and once with both regions having flat (slope < 1) APD restitution.
Results
Wavebreak and re-entry were readily initiated using the S1S2S3 protocol in tissue sheets with two regions having different APD restitution properties. Initiation occurred irrespective of whether the APD restitution slopes were steep or flat. With steep APD restitution, the range of S2S3 intervals resulting in wavebreak increased from 1 ms with S1S2 of 250 ms, to 75 ms (S1S2 180 ms). With flat APD restitution, the range of S2S3 intervals resulting in wavebreak increased from 1 ms (S1S2 250 ms), to 21 ms (S1S2 340 ms) and then 11 ms (S1S2 400 ms).
Conclusion
Regional differences in APD restitution are an arrhythmogenic substrate that can be concealed at normal heart rates. A premature stimulus produces regional differences in repolarisation, and a further premature stimulus can then result in wavebreak and initiate re-entry. This mechanism for initiating re-entry is independent of the steepness of the APD restitution curve
Geographic constraints on social network groups
Social groups are fundamental building blocks of human societies. While our
social interactions have always been constrained by geography, it has been
impossible, due to practical difficulties, to evaluate the nature of this
restriction on social group structure. We construct a social network of
individuals whose most frequent geographical locations are also known. We also
classify the individuals into groups according to a community detection
algorithm. We study the variation of geographical span for social groups of
varying sizes, and explore the relationship between topological positions and
geographic positions of their members. We find that small social groups are
geographically very tight, but become much more clumped when the group size
exceeds about 30 members. Also, we find no correlation between the topological
positions and geographic positions of individuals within network communities.
These results suggest that spreading processes face distinct structural and

Airway and parenchymal strains during bronchoconstriction in the precision cut lung slice
The precision-cut lung slice (PCLS) is a powerful tool for studying airway reactivity, but biomechanical measurements to date have largely focused on changes in airway caliber. Here we describe an image processing tool that reveals the associated spatio-temporal changes in airway and parenchymal strains. Displacements of sub-regions within the PCLS are tracked in phase-contrast movies acquired after addition of contractile and relaxing drugs. From displacement maps, strains are determined across the entire PCLS or along user-specified directions. In a representative mouse PCLS challenged with 10â4M methacholine, as lumen area decreased, compressive circumferential strains were highest in the 50 Îźm closest to the airway lumen while expansive radial strains were highest in the region 50â100 Îźm from the lumen. However, at any given distance from the airway the strain distribution varied substantially in the vicinity of neighboring small airways and blood vessels. Upon challenge with the relaxant agonist chloroquine, although most strains disappeared, residual positive strains remained a long time after addition of chloroquine, predominantly in the radial direction. Taken together, these findings establish strain mapping as a new tool to elucidate local dynamic mechanical events within the constricting airway and its supporting parenchyma
Performing the repentant lover in the courtroom: An analysis of Oscar Pistoriusâ recreation of hegemonic masculinity
Michel Foucault (1980) presented social theorists with a consideration of power as existing everywhere. Furthermore, Jonathan Heaney (J Polit Power 6:355â362, 2013) recently asserted that emotions and power should be considered conceptual counterparts. I propose that what Foucault referred in terms of the omnipresence of power refers to its deeply social connection to emotions. One emotion, in particular, romantic love, has captured the sociological imagination not only at the level of personal relationships but also in connection with capitalism, as an ideology spurring consumption and influencing the construction of discourses and places. This chapter presents an analysis of the trial of Oscar Pistorius and the analysis plays on two levels: (a) firstly, through his courtroom interactions with members of the defence, and (b) through my eyes as a viewer, witnessing the trial on television. The televised South African courtroom becomes a space for the portrayal of a power-suffused masculine identity, which is emotionally constituted through emotional control and emotional release

Neurocognitive Predictors of Response to Intervention With GraphoGame Rime
This study explores the neurocognitive predictors of response to intervention with GraphoGame Rime (GG Rime), an adaptive software game designed to aid the learning of English phonics. A cohort of 398 children (aged 6â7 years) who had participated in a recent randomised controlled trial (RCT) of GG Rime in the United Kingdom were studied. Half were randomly assigned to play GG Rime and the other half were assigned to Business As Usual (BAU). A series of pretests were given prior to the intervention to all participants, designed to measure phonological awareness skills, executive function (EF) skills and the ability to synchronise finger tapping to a rhythmic beat. Rhythmic synchronisation has been linked to reading readiness and early reading attainment, and is related to phonological awareness. Individual differences prior to the intervention in all three types of measure were significantly associated with progression through the game. Gender was also important for progression through the game, with boys progressing significantly further than girls. Vocabulary was not a predictor of progression through the game. Playing time, rhythmic synchronisation, phonological skills and EF skills did not differ by gender. Once playing time and non-verbal cognitive ability were controlled, phonological awareness, EF, rhythmic synchronisation and gender all remained significant predictors of progression through the game. In further analyses comparing these predictors, their interactions and controlling for the autoregressor of prior responsiveness to phonics instruction, phoneme awareness and EF skills were the strongest unique predictors. Analyses with the whole cohort (analysing BAU and GG children independently) showed that all neurocognitive measures contributed to progress in reading and spelling over the school year. We conclude that individual differences in phonological skills and EF skills predict which children will benefit most from computer assisted reading interventions like GG Rime. Further, boys respond better to this computerised intervention than girls. Accordingly, to be maximally beneficial to poor readers, the supplementary use of GG Rime in addition to ongoing classroom literacy instruction could be especially targeted to boys, but should be accompanied by a focus on developing both oral phonological awareness and EF skills.</jats:p
